It looks like you already did this, but the next step I would take is checking the system for any "User Added" events that would be set at the server, domain, group, and/or user level; not forgetting to look the user or group templates to see if the event is configured there. If you find two "User Added" events, doing the same thing, then I would remove/disable one of them and setup a test to see if your email is only sent once.
